Alignment Health is seeking a Field Remote Nurse Practitioner/Physician Assistant for the Jump Start Assessment program in California. The role combines in-home assessments with days working from home, under a physician-led, interdisciplinary team focused on chronic care management for seniors.

Strong communication, CA NP/PA licensure, and EMR experience are preferred. Schedule is Monday–Friday with two in-home assessment days and three remote days.

Formación

• Master’s degree from an accredited NP/PA program.
• Active California NP or PA license; NPI, DEA, and BLS are current.
• Experience with EMR systems preferred.
• Strong communication and interpersonal skills.

Responsabilidades

• Perform in-home assessments and some outpatient clinical work.
• Obtain comprehensive history, conduct physical exams, medication review, and screenings.
• Discuss end-of-life care when needed.
• Collect lab specimens in-home when appropriate.
• Identify diagnoses for chronic disease management.
• Share assessment findings with PCP to close gaps in care.
• Educate patient/family/caregiver and coordinate follow-up.
• Refer to community resources as appropriate.
• Maintain HIPAA compliance and protect PHI.
