Job summary

Alignment Health is seeking a Field Remote NP/PA to join the Care Anywhere program in California. You will conduct in-home assessments, review medications, perform health screenings, and develop care plans with the care team.

Qualifications

• Required: Master’s degree from an accredited NP/PA program.
• Active California NP or PA license and board certification.
• NPI, DEA, valid driver’s license and automobile insurance.

Responsibilities

• Conduct in-home assessments, medication reviews, and health screenings.
• Develop and coordinate care plans with the medical team.
• Partner with Regional Medical Officer and other team members to optimize patient care.
• Coordinate care with PCPs, specialists, and ancillary providers.
• Lead clinical team including nurses, health coaches, social workers, and care coordinators.
