Job summary

Alignment Health is seeking a Field NP or PA to support the Care Anywhere program, delivering medical and social care to our senior patients in their homes. Responsibilities include 2 days per week in-home assessments, 2 days remote work, medication reviews, care planning, and collaboration with PCPs, specialists, and the regional medical officer to create the best possible plans for each member.

Requirements include a Master’s NP/PA degree, active California NP/PA license with board

Qualifications

• Master’s degree from an accredited NP or PA program.
• Active California NP/PA license with board certification.
• Strong communication and time management in a fast-paced home-care environment.

Responsibilities

• Conduct in-home assessments and health screenings.
• Review medications and coordinate care plans.
• Educate patients and align care with patient goals.
• Collaborate with Regional Medical Officer and team to develop care plans.
• Coordinate care with PCPs, specialists, and ancillary providers.
• Lead clinical team including nurses and care coordinators.
