Job summary

BrightStar Care is seeking compassionate Registered Nurses to join our home health team in Laredo, TX and surrounding areas. This Part-Time or PRN position offers flexible scheduling, weekly pay, and the autonomy of field nursing while delivering patient-centered care in patients' homes.

You’ll perform skilled visits, assessments, wound care, IV therapy, and medication management, while collaborating with physicians and care teams to maintain high-quality plans of care.

Qualifications

• Current TX RN license.
• CPR certification and TB test per agency requirements.
• Experience in home health nursing preferred.

Responsibilities

• Perform skilled nursing visits in patients' homes per physician orders and Plan of Care.
• Conduct comprehensive nursing assessments and monitor patient conditions.
• Provide skilled nursing treatments including wound care, meds, injections, and other procedures.
• Manage and monitor patients with chronic illnesses or post-surgical needs.
• Perform medication reconciliation and educate patients and caregivers on medications.
• Provide patient education on disease management, infection prevention, safety, and health maintenance.
• Perform wound assessments and wound care treatments, including Wound VAC when ordered.
• Assist with IV therapy, PICC line care, catheter care, and other procedures as trained.
• Communicate with physicians and care team to ensure coordinated patient care.
• Complete accurate clinical documentation and updates to the Plan of Care.
• Report significant changes to physicians and leadership.
• Maintain compliance with policies and home health regulations.

What You'll Do

• Perform skilled nursing visits in patients' homes, providing individualized care according to physician orders and the established Plan of Care.

• Conduct comprehensive nursing assessments, monitor patients' conditions, and identify changes requiring physician notification or additional interventions.

• Provide skilled nursing treatments, including wound care, dressing changes, medication administration, injections, and other physician-ordered procedures.

• Manage and monitor patients with chronic illnesses, post-surgical needs, and other conditions requiring skilled nursing care.

• Perform medication reconciliation, assess medication effectiveness, and educate patients and caregivers on proper medication management.

• Provide patient and caregiver education on disease management, infection prevention, safety precautions, and overall health maintenance.

• Perform wound assessments and provide wound care treatments, including Wound VAC therapy when ordered and appropriately trained.

• Assist with IV therapy, PICC line care, catheter care, and other specialized nursing procedures based on patient needs, competency, and training.

• Communicate with physicians, case managers, therapists, and other healthcare professionals to ensure coordinated patient care.

• Complete accurate and timely clinical documentation, including skilled nursing visit notes, assessments, and updates to the Plan of Care.

• Report significant changes in patient conditions to the appropriate physician and agency clinical leadership.

• Maintain compliance with agency policies, infection control practices, and applicable home health regulations.
