• Minimum 3 years in healthcare/population health.
• Strong communication skills.
• Ability to build trust with a wide range of stakeholders.

• Proficient in Excel and Google Apps.
• Bachelor’s degree is required; US work authorization.
• Scribing experience is a plus.

Responsibilities

• Lead strategic planning and refinement of Population Health programs in the region.
• Collaborate with regional leaders and center care teams to support the Oak Street care model.
• Develop scalable clinical documentation and avoidable admission prevention strategies.
• Partner with central office to design strategy and program development for documentation and cost management.
• Lead analytics, insights, and project management for population health execution.
• Recruit and engage medical scribe cohorts in the region.
• Be accountable for population health metrics including quality and utilization.
