Job summary

Oak Street Health is seeking a Population Health Director to lead local programs across our Brooklyn centers. You will design and implement health initiatives, align care teams, and drive metrics for quality and cost.

The role requires strong partnerships with clinicians and operations to deliver rapid, impactful improvements. The ideal candidate has 7–10 years in healthcare/population health, with a Bachelor’s degree and preferred MBA/MPH.

Qualifications

• Bachelor's degree required; graduate degree preferred (MBA, MPH).
• 7–10 years of prior relevant experience in healthcare/population health or related fields.
• Ability to build trust with a wide range of stakeholders; strong leadership and communication skills.
• Analytical and data savvy with ability to develop business cases, ROI, and financial analyses.

Responsibilities

• Own the execution and refinement of Population Health programs in the local division.
• Collaborate with central office leaders to assist with strategy and program design.
• Partner with regional Care Teams and Operations to implement clinical and business priorities.
• Drive key population health metrics including quality, utilization, and cost measures.
