Job summary

You will drive strategic planning, monitor key metrics, and develop scalable documentation and cost-management initiatives across Oak Street Health centers. Exceptional leadership and analytical skills are essential.

Qualifications

• 5+ years of healthcare/population health leadership experience.
• Excellent communication and stakeholder management.
• Proven track record in data-driven decision making.
• Strong leadership and cross-functional collaboration.
• Ability to manage multiple priorities and deliver results.
• Graduate degree preferred (MBA/MPH) but not required; Bachelor’s required.

Responsibilities

• Own strategic planning and execution of Population Health programs in the region.
• Collaborate with regional leaders and center care teams to support care model.
• Develop scalable strategies for accurate documentation and cost management.
• Design program initiatives with central office partners for revenue and outcomes.
• Analyze data to monitor performance and drive improvements.
• Recruit, retain and engage the medical scribe cohort within the region.
