Job summary

21st Century Home Health Services is seeking an entry‑level Health Field Physical Therapist to build core clinical skills in a home health setting in the Dixon area. You will perform follow-up home visits under the supervision of a Physical Therapist Case Manager, focusing on hands-on therapy and patient education.

Ideal candidates hold a California PT license, BLS/CPR, and a PT degree, with a preference for some home health experience.

Qualifications

• Current, unrestricted California Physical Therapist license.
• BLS/CPR certification is required.
• Valid CA Driver’s License with clean record.
• Graduate of an accredited Physical Therapy program.
• One year of PT experience preferred; new grads may be considered.
• Home health experience is highly desirable.
• Ability to work as part of an interdisciplinary team.
• Familiarity with case management principles.
• Comfortable with electronic documentation and mobile health tools.

Responsibilities

• Provide hands-on therapeutic interventions including gait training and therapeutic exercises.
• Conduct in-home patient evaluations and assessments.
• Implement plans of care under supervision of a PT Case Manager.
• Document patient care within 24–48 hours per plan of care.
• Educate patients and families on mobility, safety, and adaptive equipment.
• Collaborate with the PT Case Manager for care plan updates.
• Participate in interdisciplinary team conferences and coverage rotations.
