Job summary

21st Century Home Health Services is seeking an entry‑level Health Field Physical Therapist to build core clinical skills in a home health setting and support operations. The position focuses on follow-up visits with strong leadership mentorship and development opportunities.

You will work under a Physical Therapist Case Manager to deliver hands-on therapy, conduct home evaluations, and help with plan of care while meeting productivity and documentation standards.

Qualifications

• Current, unrestricted California PT license.
• Current BLS/CPR certification.
• Valid California driver’s license with clean driving record.
• Graduate of an accredited Physical Therapy program.
• One year of PT experience preferred; new grads considered.
• Home health experience desirable.
• Experience with high-acuity patients a plus.
• Ability to work in an interdisciplinary team (RN, OT, ST, MSW, HHA).
• Familiarity with case management principles.
• Strong clinical judgment and communication.
• Comfortable with electronic documentation and mobile tools.
• Self-direction and time management in fast-paced settings.
• Commitment to annual compliance training and continuing education.

Responsibilities

• Provide hands-on therapeutic interventions in home health setting (gait training, balance, neuromuscular re-education, transfers, therapeutic exercises, manual therapy, pain management).
• Support patients with assistive devices and home safety equipment.
• Conduct in-home patient evaluations and assessments.
• Develop individualized plans of care under supervision of PT Case Manager.
• Document patient care within 24–48 hours per plan of care and compliance standards.
• Educate patients and caregivers on mobility, safety, and equipment use.
• Collaborate with PT Case Manager to prepare treatments and update care plans.
• Coordinate with RN, OT, ST, MSW, HHA for interdisciplinary interventions.
• Maintain privacy and dignity in documentation and care.
• Participate in weekly interdisciplinary case conferences and on-call rotations.
