Job summary

21st Century Home Health Services seeks an entry-level Field Physical Therapist to build core clinical skills in a home health setting, focusing on follow-up visits with strong leadership support. You will work under the supervision of a Physical Therapist Case Manager, covering the El Cerrito area and surrounding counties.

The role emphasizes direct patient care, documentation, and collaboration with an interdisciplinary team to ensure high-quality outcomes and safety in the home setting.

Qualifications

• Current, unrestricted California Physical Therapist (PT) License.
• Current BLS/CPR Certification.
• Valid CA Driver’s License with a clean driving record.
• Graduate of an accredited Physical Therapy program.
• One year of experience as a Physical Therapist preferred; new grads considered.
• Prior home health experience desirable.

Responsibilities

• Provide hands-on therapeutic interventions in home health, including gait, balance, and neuromuscular re-education.
• Manage patients with assistive devices and safety equipment.
• Conduct detailed patient evaluations in the home environment.
• Develop and implement plans of care under PT Case Manager supervision.
• Document care within 24–48 hours per plan and compliance standards.
• Educate patients and families on mobility, exercises, safety, and adaptive equipment.
• Collaborate with PT Case Manager and interdisciplinary team.
• Participate in on-call or weekend coverage as required.
