Job summary

BayCare HomeCare seeks a Field Physical Therapist for home health in Largo, FL. You will evaluate patients, develop customized treatment plans, and lead care teams to deliver in-home services.

This role requires a Florida PT license, 1 year of PT experience, and BLS. You’ll work a Full-Time schedule with weekday hours and no on-call obligations, serving Largo and surrounding areas.

Qualifications

• Active Florida Physical Therapy license.
• 1 year Physical Therapist experience.
• BLS (Basic Life Support).
• Driver's License (Florida).
• Preferred Master's Degree in Physical Therapy.
• Master's degree in Physical Therapy preferred.

Responsibilities

• Evaluates and develops treatment programs for individual patients.
• Provide clinical direction to Physical Therapy Assistants.
• Plan and manage home health services to meet patient needs.
• Coordinate with multi-disciplinary team for patient care.
• Communicate caseload and patient issues to supervisor.
• Develop and carry out discharge planning with patient and family.
• Review patient visits and adjust care plans weekly.
• Reviews patients visits weekly and provides plan to Home Care Supervisor and scheduler, including anticipated weekend visits.
• Informs scheduler of any schedule changes and communicates caseload and patient care issues to Home Care Supervisor.

Shift Hours: 8:00 a.m. - 5:00 p.m., Monday-Friday

On Call: No

Home visits in assigned service area: Largo, St. Petersburg, Pinellas Park - surrounding areas assigned by zip code.

HomeCare Physical Therapist - Responsibilities include :

• Evaluates and develops specific treatment programs for individual patients based on their needs and in accordance with the principles and practices of physical therapy.
• Responsible for providing the appropriate level of clinical direction for Physical Therapy Assistants.
• Plans, implements, manages and evaluates the provision of both professional and ancillary home health services (direct care and case management) to ensure that all patients needs are met and quality care provided in accordance with Federal, State, JCAHO and program guidelines.
• Manages the activities of the multi-disciplinary team providing care to the patient.
• Monitors the provision of therapeutic services through written and verbal communication with all disciplines.
• Develops, implements, and carries out a discharge plan in conjunction with the patient, family/caregiver and members of the health care team.
• Interprets agency policy to patients/families.
• Reviews patients visits weekly and provides plan to Home Care Supervisor and scheduler, including anticipated weekend visits.
• Informs scheduler of any schedule changes and communicates caseload and patient care issues to Home Care Supervisor.

HomeCare Physical Therapist Requirements:

• Bachelor's Degree Physical Therapy
• Physical Therapist State of Florida License
• 1 year Physical Therapist experience
• BLS (Basic Life Support)
• Driver's License (Florida driver's license is required within 30 days of hire)
• Preferred Master's Degree Physical Therapy
