Job summary

BayCare HomeCare in Dunedin, FL is seeking a Home Health Physical Therapist to deliver patient-centered care in patients' homes. The role involves evaluating patients, planning and implementing PT programs, and coordinating with an interdisciplinary team. Requires Florida PT license, BLS, and valid driver's license; at least 1 year of experience.

Flexible schedule and opportunities for growth are offered. Work hours are 8:00 a.m.–5:00 p.m., Monday–Friday, with occasional weekends and home visits

Qualifications

• Bachelor's degree in Physical Therapy required.
• Florida Physical Therapist license required.
• Minimum 1 year of Physical Therapist experience.
• Basic Life Support (BLS) certification required.
• Driver's license (Florida) required within 30 days of hire.
• Preferred Master's Degree Physical Therapy.

Responsibilities

• Evaluates and develops specific treatment programs for individual patients based on their needs and in accordance with physical therapy principles.
• Provides clinical direction for Physical Therapy Assistants.
• Plans, implements, manages and evaluates provision of home health services to ensure patient needs are met and quality care.
• Manages activities of the multi-disciplinary team providing care to the patient.
• Monitors provision of therapeutic services through written and verbal communication with all disciplines.
• Develops discharge plans with patient and health care team.
• Interprets agency policy to patients and families.
• Reviews patient visits weekly and communicates schedule changes to Home Care Supervisor.

HomeCare Physical Therapist - Responsibilities include :

• Evaluates and develops specific treatment programs for individual patients based on their needs and in accordance with the principles and practices of physical therapy.
• Responsible for providing the appropriate level of clinical direction for Physical Therapy Assistants.
• Plans, implements, manages and evaluates the provision of both professional and ancillary home health services (direct care and case management) to ensure that all patients needs are met and quality care provided in accordance with Federal, State, JCAHO and program guidelines.
• Manages the activities of the multi-disciplinary team providing care to the patient.
• Monitors the provision of therapeutic services through written and verbal communication with all disciplines.
• Develops, implements, and carries out a discharge plan in conjunction with the patient, family/caregiver and members of the health care team.
• Interprets agency policy to patients/families.
• Reviews patients visits weekly and provides plan to Home Care Supervisor and scheduler, including anticipated weekend visits.
• Informs scheduler of any schedule changes and communicates caseload and patient care issues to Home Care Supervisor.

HomeCare Physical Therapist Requirements:

• Bachelor's Degree Physical Therapy
• Physical Therapist State of Florida License
• 1-year Physical Therapist experience
• BLS (Basic Life Support)
• Driver's License (Florida driver's license is required within 30 days of hire)
• Preferred Master's Degree Physical Therapy
