F/T or Per Diem - New RN Graduates Welcome!

Able Health Care Service is a leading home care provider offering Home Health Aide/Personal Care Aide services to patients in need.

Our home care workers support and assist patients with activities of daily living. Able Health Care has been in business for 48 years and highly values our clinical team.

We are seeking to hire F/T or P/T (Per Diem) RNs to visit patients' homes to perform assessments and supervise Home Health Aides.

Field RN duties include:

• Assess patients at the start of care to develop individualized care plans tailored to each patient's needs and goals.
• Provide patient education and support to promote self-management.
• Supervise Able’s Home Health Aides, ensuring they understand their roles and tasks, and establish their competency.
• Reassess patients periodically to monitor health status and supervise HHA to ensure proper care.
• Document all status changes according to agency policies.

Qualifications:
Valid RN license in New York State.
New RN graduates are welcome; training and support are provided.

Compensation:
Full Time: $95K - $125K/year
Per Diem: $85 - $120 per visit, depending on service type.
