Job summary

Advantmed is a leading provider of risk adjustment and quality improvement solutions. We are seeking a dedicated Nurse Practitioner to join our full-time provider network for in-home wellness and risk adjustment assessments across the state.

The NP will ensure accurate documentation, support high-quality value-based care, and travel to members' homes within Oregon with a focus on primary care and chronic conditions.

Qualifications

  • Active NP license in Oregon.
  • Minimum 3 years of direct patient care experience (primary care, adult/geriatric preferred).
  • EMR proficiency is desired.
  • Bilingual proficiency is an advantage.

Responsibilities

  • Perform in-home wellness and risk adjustment assessments.
  • Travel to members' homes for comprehensive health evaluations.
  • Conduct annual wellness visits and health assessments for chronic conditions.
  • Review medical history, exams, vitals, and medications; include preventive screenings.
  • Educate patients on ongoing health monitoring and disease management.
  • Collaborate with care team to close quality care gaps and document findings.

Primary Purpose

Advantmedisseekinga dedicated and compassionate Nurse Practitioner (NP) to join our full-time provider network. In this role, you will performin-homewellness and risk adjustment assessments for diverse patient populations with varying healthcare needs. Your work will ensureaccurate, comprehensive documentation and support high-quality,value-basedclinical care across the state.

Key Responsibilities
In-homeClinical Assessments
  • Travel to members’ homes and conduct comprehensive health evaluations
  • Conduct Annual Wellness Visits and health assessments for members with chronic conditions.
  • Perform:
  • Medical history and physical examinations
  • Review of systems
  • Vital signs collection
  • Medication review and reconciliation
  • Possible preventive health screenings
  • Hemoglobin A1C
  • Spirometry
  • KED (Kidney Health Evaluation)
  • DRE (Diabetic Retinal Exam)
  • FIT
  • Bone Density
Patient Education & Engagement
  • Deliver preventive health education and counseling opportunities.
  • Educate patients on ongoing health monitoring, medication adherence, and chronic condition management.
  • Support informed decision-making and ensure true, informed consent.
  • Engage in goalsofcare discussions whenappropriate.
Care Coordination & Quality Support
  • Identifyclinically relevant diagnoses for care management and risk adjustment documentation.
  • Communicate findings used to generate postvisit summaries for the primary care provider.
  • Identifyand help close quality care gaps (e.g., screenings, labs, follow-ups).
  • Collaborate enthusiastically with the broader care team to enhance clinical quality and patient experience.
Travel Requirements
  • Full-time providers support the entire state of Oregon.
  • Daily travel expectationsremainwithin a 55-mile radius of the assigned home location.
  • Statewide travel will be scheduled with a minimum of two weeks’ notice.
Compliance & Professional Standards
  • Adhere to HIPAA regulations and safeguard protected health information (PHI).
  • Maintainaccurate,timely, and compliant EMR documentation.
  • Performadditionalduties as assigned in alignment with organizational goals.
Qualifications
  • Active, unencumbered Nurse Practitioner (NP) license in Oregon.
  • Minimum 3 years of direct patient care experience (primary care, adult/geriatric preferred).
  • EMRproficiencyis desired.
  • Priorin-homerisk assessment experience is a plus but notrequired.
