Job summary

Advantmed seeks a licensed Nurse Practitioner to perform in-home wellness and risk-adjustment assessments for Medicare members and at-risk populations in a PRN 1099 framework. Flexible weekdays with no evenings, weekends, or on-call; travel within a 55-mile radius of the assigned county, with paid mileage and training provided.

In this role you will conduct visits, document in EMR, educate members on preventive care, and coordinate with a dedicated team to support value-based care.

Qualifications

• Active, unencumbered NP license in the applicable state.
• 3+ years patient care experience preferred.
• Comfortable with EMR systems and independent work.
• In-home or risk-adjustment experience preferred.
• Willing to obtain additional licensure if needed (supported by Advantmed)

Responsibilities

• Conduct in-home Annual Wellness Visits and chronic condition assessments.
• Perform focused histories, exams, vitals, med reviews, and screenings.
• Screenings: HbA1c, Spirometry, KED, DRE, FIT.
• Educate members on preventive care and chronic disease management.
• Document visits in the EMR to support value-based care and risk adjustment.

What You’ll Do

Important Role Expectations

This is a 1099 PRN role requiring consistency and reliability.

Minimum commitment: ~ 30 hours per month , ongoing - Not intended for casual or very short‑term engagement.

• Daily travel expectations remain within a 55-mile radius of the assigned homelocation.

NP Qualifications:

• Must have a valid unencumbered NP License for the state you will be working in
• This role requires travel up to a maximum 55-mile radius originating in the assigned job posting county
• Preference is given to weekday schedules
• Preference is given to standard working hour schedules
• Previous in-home risk assessment experience preferred
• Previous 1099 (PRN) experience is preferred but not required
• 3 years patient care experience preferred (primary care/adult/geriatric, EMR)
• May be requested to obtain additional NP licensure supported by Advantmed
• Access to reliable transportation that will enable you to travel tomembers’homes within a designated area
• Strong ability to work within our EMR system
• Ability to work independently
• Bilingual is a plus
