Job summary

Paradigm is hiring a Field Nurse Case Manager to cover Elizabeth and nearby communities, including Edison, Somerville, Newark, and New Brunswick. This field/home-based role requires travel up to 2 hours one-way, visiting doctors' offices, hospitals and other sites.

The role involves medical case management of work-related injuries: assessment, planning, coordination, implementation and evaluation, with collaboration among insurers, providers, attorneys, employers and employees.

Qualifications

Active NJ RN license required.
NY RN license preferred; must obtain within 60 days of hire.
Experience in workers' comp case management preferred.
Certifications CCM/CRRN/CDMS/CRC preferred; required within two years of hire.
Strong organization, timeline adherence, PC/tech and communication skills.
Able to operate autonomously with minimal oversight.
Skilled in patient education.
Valid Driver’s License with clean driving history.

Responsibilities

Manage medical case work-related injuries from assessment through evaluation.
Coordinate with insurance carriers, medical providers, attorneys, employers and employees.
Travel to doctors' offices, hospitals and other locations; may have multiple appointments per day.
Monitor progress and report results to the employer and insurance carriers.
Cover Elizabeth and nearby communities (Elizabeth, Edison, Somerville, Newark, New Brunswick).
