Job summary

Caringhand Home Health Solutions, Inc. is seeking licensed practical nurses (LPNs) to provide in-home care across California. The role involves per diem visits, travel to patient homes, and carrying out individualized care plans to support safe recovery and day-to-day self-care.

We value compassionate, patient-first care delivered in a family-oriented atmosphere. A California LPN license, CPR certification, and reliable vehicle are required. Mileage reimbursement is provided for patient visits.

Qualifications

• LPN/LVN license in California is mandatory.
• 3+ years of nursing experience; home health or med/surg background preferred.
• Current CPR certification with AHA.
• Valid driver’s license and reliable insured vehicle required for home visits.

Responsibilities

• Provide direct patient care in accordance with the state Nurse Practice Act.
• Implement and follow individualized care plans established by the RN.
• Educate patients and families on self-care techniques and diagnosis-related information.
• Document care accurately and in a timely manner.
• Collaborate with the care team and communicate patient needs to the RN or Clinical Supervisor.
• Use supplies safely and adhere to aseptic technique.
