Job summary

Caringhand Home Health Solutions, Inc. is seeking a field-based Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) to provide in-home patient care across Placerville, CA.

Per visit compensation ranges from $45 to $55, with duties centered on implementing care plans and patient education. Travel to patients’ homes is required; the role offers flexible scheduling, potential for full-time conversion based on performance, and additional benefits like paid sick days and bereavement

Qualifications

• Graduate of an accredited practical or vocational nursing program.
• Active LPN or LVN license in California.
• CPR certification with AHA.

Responsibilities

• Provide direct patient care in accordance with the state Nurse Practice Act.
• Implement and follow individualized care plans established by the RN.
• Educate patients and their families on self-care techniques and diagnosis-related information.
• Document care accurately and in a timely manner.
• Collaborate with the care team — including OT, PT, HHA, and other disciplines — and communicate patient needs to the RN or Clinical Supervisor.
• Use supplies and equipment safely, adhering to aseptic technique.
• Perform other duties as required for patient services.
