Job summary

Premier Home Health Providers Inc. seeks an experienced Licensed Practical Nurse (LPN) or LVN to provide direct patient care in patients' homes in California.

You will follow individualized care plans, educate families, document care, and collaborate with the care team to support safe, quality recovery. This per diem role offers a competitive per-visit rate ($45-$55), mileage reimbursement, flexible scheduling, and the potential to convert to full-time based on performance.

Qualifications

• Graduate of an accredited practical or vocational nursing program.
• Active LPN or LVN license in California.
• 3+ years of nursing experience; home health or med/surg background preferred.
• Current CPR certification with AHA.

Responsibilities

• Provide direct patient care in accordance with the state Nurse Practice Act.
• Implement and follow individualized care plans established by the RN.
• Educate patients and families on self-care techniques and diagnosis-related information.
• Document care accurately and in a timely manner.
• Collaborate with the care team — including OT, PT, HHA, and other disciplines — and communicate patient needs to the RN or Clinical Supervisor.
• Use supplies and equipment safely, adhering to aseptic technique.
• Perform other duties as required for patient services.
