Responsibilities include documenting case notes, coordinating housing supports, attending meetings, and supporting clients through enrollment and discharge while collaborating with partners. Qualifications

• Administrative and clinical experience in behavioral health or related fields.
• Ability to work with mentally ill and homeless individuals.
• Availability to work varied hours, including evenings and weekends.
• Valid Washington State driver’s license; proof of insurance; background check cleared.

Responsibilities

• Follow documentation processes and procedures for FHARPS consumers.
• Provide direct service to FHARPS program participants.
• Attend FHARPS admin monthly meetings.
• Discharge clients in Avatar and PDAMS per supervisor guidance.
• Maintain electronic client files and housing program documents in OneDrive.
• Coordinate with property owners for maintenance and housing turnover.
• Support FHARPS communications and respond to client inquiries.
