Job summary

Ohio's Hospice, Inc. is seeking a Hospice House PCS to provide direct patient care in Dayton, OH. This full-time position supports families and patients, traveling to homes and care facilities as part of the Zanesville service region.

The role requires a CNA or willingness to train, a valid driver’s license, and comfort documenting patient care in electronic medical records. Competitive salary, mileage reimbursement, and ongoing education are offered.

Qualifications

• High School Diploma or GED required or equivalent.
• Certified Nurse Aide preferred; will train internally.
• CHPNA certification preferred; Ohio's Hospice supports certification growth.
• Proficient computer skills to document patient care in EMR.
• Valid Ohio driver’s license with safe driving record required.
• Background check and 10-panel drug screen required.

Responsibilities

• Deliver direct patient care in a Hospice setting.
• Document patient care accurately in electronic medical records.
• Travel to patient homes and care facilities throughout the service region.
• Collaborate with interdisciplinary team to support patients and families.
• Occasionally float to Middletown and Troy IPU locations as needed.
