Qualifications

• Proficiency with Windows, Microsoft Office; EMR experience preferred.

Responsibilities

• Care Management and Service Delivery in patients' homes or facilities, 90% of duties.
• Develop and implement comprehensive care plans addressing patient needs.
• Coordinate with team members for changes in patient condition.
• Provide patient and caregiver education as needed.
• Oversee plan of care implementation and eval of outcomes.
