Job summary

Ohio Living is seeking an RN Field Staff to coordinate and deliver quality home health and hospice services across Ohio. You will assess patients, develop care plans, and ensure compliance with state and federal guidelines while collaborating with interdisciplinary teams.

Responsibilities include ongoing care evaluation, patient and caregiver education, and timely communication with care coordinators. A valid Ohio RN license and willingness to travel are required.

Qualifications

• Current unencumbered license for the state of Ohio to practice as a Registered Nurse (RN) required.
• One year experience in acute care or medical-surgical nursing preferred.
• Experience in a Medicare-certified home health agency preferred.
• Proficiency with Windows, Microsoft Office (Word, Excel, PowerPoint), and the internet required.

Responsibilities

• Care Management and Service Delivery - 90%: Assess patients in homes and healthcare facilities; develop and implement care plans; coordinate with team leaders and other providers.
• Quality and Compliance Management - 5%: Maintain accurate patient records; participate in quality improvement projects; stay updated on regulatory requirements.
• People Management and Development - 5%: Coach and guide LPN Field Staff and Home Health Aides; promote performance standards; participate in training and orientation programs.
• Oversee plan of care implementation; Prepare for IDG by reviewing care plans and reconciling medications; ensure timely communication with team members.
