Job summary

Reema Health is seeking a Community Health Navigator to build genuine relationships with members and guide them toward greater engagement in their own healthcare. You'll work daily with Reema's technology platform to reach members, schedule visits, and document progress in their care plans.

Qualifications

• Requires 18+ years of age or older.
• Available to work Monday–Friday during core business hours.
• Valid driver’s license, insurance, and reliable transportation (field-based).
• Pre-employment drug screening and background check required.
• Comply with HIPAA and patient confidentiality requirements.
• Proficient with technology-based tools and systems daily.

Responsibilities

• Conduct phone and in-person outreach to enroll members in Reema's care model.
• Manage a caseload of 80-120 members and support colleagues during high workload periods.
• Assess member needs, including social determinants of health, to plan care.
• Build trusted relationships and guide members toward goal-oriented actions.
• Coordinate referrals and schedule appointments, ensuring attendance.
• Organize workload to meet monthly goals and daily metrics.
• Engage members using phone, text, email, and in-person visits.
