JOB DESCRIPTION

Job Summary

The Care Connections Nurse Practitioners focus on screening and preventive primary care services delivered in the home, community, and nursing facility settings. They provide care in environments where patients feel most comfortable, including home, nursing facilities, and "pop-up" clinics.

The Nurse Practitioner will primarily work in non-clinical settings, providing medical care to diverse patient populations. Some programs may target specific groups such as adults, geriatrics, pediatrics, or women’s health.

Responsibilities include performing comprehensive medical assessments, ordering diagnostic tests, formulating treatment plans, consulting with specialists as needed, and maintaining proper documentation.

Job Duties

  1. Provide general medical care and care coordination for various patient groups, including adults, women’s health, pediatrics, and geriatrics.
  2. Conduct comprehensive evaluations, including history and physical exams, to identify gaps in care and perform preventive assessments.
  3. Address both chronic and acute primary care issues, assessing medical urgency.
  4. Establish and document accurate medical diagnoses.
  5. Seek specialty consultations when appropriate.
  6. Order and interpret diagnostic tests judiciously, working within resource limitations.
  7. Recognize when patient needs exceed their scope and refer to physicians accordingly.
  8. Create and implement medical care plans.
  9. Schedule patient visits as appropriate.
  10. Coordinate post-discharge care to reduce hospital readmissions and ER visits.
  11. Perform face-to-face visits in various settings, including home and nursing facilities, and via alternative modalities as needed.
  12. Order targeted laboratory tests for specific populations.
  13. Participate in on-call coverage to manage urgent lab and pharmacy inquiries.
  14. Engage in community-based "Pop Up Clinics" to build community relationships and address healthcare gaps.
  15. Drive up to 120 miles daily within the region, with occasional longer trips or overnight stays for special projects.
  16. Obtain and maintain licenses in multiple states based on business needs.
  17. Collaborate with colleagues to optimize work practices.
  18. Participate in regional meetings and activities.
  19. Prescribe medications and perform procedures as appropriate.
  20. Document care accurately and timely in electronic medical records.
  21. May need to walk stairs while carrying equipment up to 50 lbs.

JOB QUALIFICATIONS

EDUCATION:

Master's degree in family health from an accredited nursing program.

EXPERIENCE/KNOWLEDGE, SKILLS & ABILITIES:

Proficiency in computer skills, including Word, Excel, and EMR systems.

LICENSE, CERTIFICATION, AND ASSOCIATION:

  • Active, unrestricted national certification (e.g., AANP, ANCC).
  • Current state license as a Family Nurse Practitioner.
  • Basic Life Support (BLS) certification.
  • Unrestricted driver’s license.

PREFERRED QUALIFICATIONS

  • 3-5 years of experience as a Registered Nurse or Nurse Practitioner, preferably in home or community health.
  • Experience managing chronic conditions in home health settings.
  • Experience serving underserved populations facing socioeconomic barriers.
  • Fluency in additional languages is a plus.
  • Skills in immunization and point-of-care testing.
