JOB DESCRIPTION

Job Summary

The Care Connections Nurse Practitioners focus on screening and preventive primary care services delivered in the home, community, and nursing facility settings. They provide needed care in environments where patients feel most comfortable and receptive, including home, nursing facilities, and "pop-up" clinics.

The Nurse Practitioner will primarily work in non-clinical settings, providing medical care to diverse patient populations. Some programs may focus on specific groups, such as adults, geriatrics, pediatrics, or women’s health.

Responsibilities include performing comprehensive medical assessments, ordering tests and procedures for diagnostics, formulating treatment plans, consulting with specialists as needed, and ensuring proper documentation.

Job Duties

  1. Provide general medical care and care coordination for various patient groups, including adults, women’s health, pediatric, and geriatric populations.
  2. Conduct comprehensive evaluations, including history and physical exams, to identify gaps in care and perform preventive assessments.
  3. Address both chronic and acute primary care issues, assessing medical urgency.
  4. Establish and document accurate medical diagnoses.
  5. Seek specialty consultations when appropriate.
  6. Order and interpret diagnostic tests judiciously, working within resource-limited environments.
  7. Recognize when patient needs exceed scope and require physician oversight.
  8. Create and implement medical care plans.
  9. Schedule patient visits as needed.
  10. Coordinate post-discharge care to reduce readmissions and ER visits.
  11. Perform face-to-face visits in various settings, including home and nursing facilities, and via alternative modalities as needed.
  12. Order targeted laboratory tests for specific populations.
  13. Participate in on-call coverage to triage urgent results and inquiries, developing appropriate care plans.
  14. Engage in community-based "Pop Up Clinics" to build community relationships and address healthcare gaps.
  15. Travel up to 120 miles daily within the assigned region; overnight stays may be required for special projects.
  16. Obtain and maintain licenses in multiple states based on business needs.
  17. Collaborate with colleagues to develop best practices and participate in regional meetings.
  18. Prescribe medications and perform procedures as appropriate.
  19. Document care timely in electronic medical records.
  20. Occasionally perform physical tasks, such as walking stairs with equipment.

JOB QUALIFICATIONS

EDUCATION:

Master's degree in family health from an accredited nursing program.

EXPERIENCE/SKILLS:

Proficiency in computer skills, including Word, Excel, and EMR systems.

LICENSES & CERTIFICATIONS:

  • Active, unrestricted national certification (e.g., ANCC, AANP).
  • Current state license as a Family Nurse Practitioner.
  • Basic Life Support (BLS) certification.
  • Unrestricted driver’s license.

PREFERRED QUALIFICATIONS

  • 3-5 years of experience as a Registered Nurse and/or Nurse Practitioner, especially in home or community health.
  • Experience managing chronic conditions in home health settings.
  • Experience working with underserved populations.
  • Additional language skills are a plus.
  • Immunization and point-of-care testing skills.
