JOB DESCRIPTION
Job Summary

The Care Connections Nurse Practitioners focus on screening and preventive primary care services delivered in the home, community, and nursing facility settings. They provide needed care in environments where patients feel most comfortable and are most receptive, including home, nursing facilities, and "pop-up" clinics.

The Nurse Practitioner will primarily work in non-clinical settings, providing medical care to diverse patient populations. Some programs may focus on specific groups such as adults, geriatrics, pediatrics, or women’s health.

Responsibilities include performing comprehensive medical assessments, ordering diagnostic tests, formulating treatment plans, consulting with specialists as needed, and maintaining proper documentation.

Job Duties
  1. Provide general medical care and care coordination for various patient groups, including adults, women’s health, pediatrics, and geriatrics.
  2. Conduct comprehensive evaluations, including history and physical exams, to identify care gaps and perform preventive assessments.
  3. Address both chronic and acute primary care issues, assessing medical urgency.
  4. Establish and document accurate medical diagnoses.
  5. Seek specialist consultations when appropriate.
  6. Order and interpret diagnostic tests judiciously, especially in resource-limited environments.
  7. Recognize when patient needs exceed scope of practice and escalate to physicians.
  8. Create and implement medical care plans.
  9. Schedule patient appointments as needed.
  10. Coordinate post-discharge care to reduce readmissions and ER visits.
  11. Perform face-to-face visits in various settings, including home and nursing facilities, and via alternative modalities when appropriate.
  12. Order targeted laboratory tests for specific populations.
  13. Participate in on-call coverage for urgent cases and develop appropriate care plans.
  14. Engage in community outreach through "Pop Up Clinics" to improve healthcare access.
  15. Travel up to 120 miles daily within the assigned region, with potential overnight stays for special projects.
  16. Obtain and maintain licensure in multiple states as needed.
  17. Collaborate with colleagues to develop best practices and participate in regional meetings.
  18. Prescribe medications and perform procedures as appropriate.
  19. Ensure timely documentation in electronic medical records.
  20. Occasionally perform physical activities such as walking stairs with equipment.
Job Qualifications
Required Education

Master's degree in family health from an accredited nursing program.

Required Experience, Skills & Abilities

Proficiency in computer skills, including Word, Excel, and Electronic Medical Records.

Required Licenses, Certifications & Associations
  • Active and unrestricted national certification from the American Academy of Nurse Practitioners or the American Nurses Credentialing Center.
  • Current state-issued license as a Family Nurse Practitioner.
  • Basic Life Support (BLS) certification for Healthcare Professionals.
  • Unrestricted driver’s license.
Preferred Education & Experience
  • 3-5 years of experience as a Registered Nurse or Nurse Practitioner, preferably in home or community health settings.
  • Experience managing chronic conditions in home health.
  • Experience serving underserved populations facing socioeconomic barriers.
  • Fluency in additional languages is a plus.
  • Skills in immunization and point-of-care testing.
