CVS Health Corporation seeks a full-time Community Health Worker in Louisiana (Region 7). The role involves extensive in-state travel (70%), home visits, and education to connect members with healthcare and social services.

You will work under supervision to improve health outcomes and member engagement. Responsibilities include implementing care plans, coordinating with providers, and documenting activity in the EHR.

Qualifications

• Travel 70% of the time with reliable transportation.
• 3-5 years' relevant work experience.
• 1-2 years' experience providing clinical support services.
• 1 year of community outreach experience in LA.
• Experience with clinical clients (physical and/or behavioral health).
• Proficient in MS Office and EHR systems.

Responsibilities

• Conduct home visits to reinforce care plans and address health needs.
• Educate providers on social determinants of health affecting care.
• Build relationships with members, providers and team members from diverse backgrounds.
• Identify and reduce cultural and socio-economic barriers to care.
• Assist with care plan implementation and health/social management planning.
• Help members obtain social services, appointments, and program applications.
• Support medication adherence and review medications with members.
• Teach disease self-management and track symptoms.
• Accompany members to appointments as needed.
• Enter and maintain records in the EHR and complete program documentation.
• Travel extensively to client homes and outreach locations.
