Qualifications

Responsibilities

• Conduct home visits with members to reinforce care plans and reduce hospitalizations
• Educate providers on social determinants of health affecting care plans
• Build relationships with members, providers, and staff across diverse backgrounds
• Assist with care plan implementation and develop health and social management plans
• Document activities in electronic health records and generate reports
• Travel extensively to client homes and other outreach sites
