Qualifications

• Requires BA/BS degree and a minimum of 1 year of experience working directly with people related to the CFSP population or similar community-based services.
• Two (2) years of experience working directly with individuals served by child welfare is preferred.

Responsibilities

• Provide integrated whole-person care management under the CFSP model, coordinating across physical health, behavioral health, and social supports.
• Collaborate with county Child Welfare Workers to align health care with permanency goals.
• Coordinate care with a multidisciplinary team to ensure seamless, plan-driven service delivery for Medicaid-enrolled families.
• Conduct telephonic or in-person assessments to identify and manage member program needs.
• Identify members who may require expanded services and connect them with appropriate resources.
