Job summary

BJC Behavioral Health in Missouri is seeking a full-time Field Case Manager to deliver crisis-based therapy, family therapy, and behavioral interventions to individuals and families across St. Louis City and County.

You will travel to clients’ homes, coordinate services, and document care in the charting system. Requirements include a Master’s Degree in Human Services or related field and a valid driver’s license.

Qualifications

• Master's degree in Human Services or related field.
• Valid Driver's License.

Responsibilities

• Completes timely documentation of services describing need, intervention, treatment plan, provider, date, time and setting, and client response.
• Contact individuals or referrals after missed appointments to re-engage and support recovery.
• Monitor services to assess progress toward outcomes and escort clients to needed services.
• Coordinate treatment and rehabilitation needs with medical, social and educational supports.
