Job summary

Molina Healthcare is seeking a Field Care Manager to support Medicaid members in the Laredo service delivery area. You will conduct member assessments and develop care plans in collaboration with a multidisciplinary team to coordinate LTSS and ensure quality, cost-effective care.

Ideal candidates have 2+ years of healthcare experience, strong Microsoft Office skills, and a valid driver’s license. Travel up to 25-40% may be required depending on state requirements.

Qualifications

• Minimum 2 years of health care experience, including LTSS exposure and care management experience.
• Valid and unrestricted driver’s license with reliable transportation for travel requirements.
• Proficiency with Microsoft Office suite (Outlook, Excel, Teams) and ability to navigate multiple online portals.

Responsibilities

• Completes comprehensive member assessments within regulated timelines, including in-person home visits.
• Facilitates waiver enrollment/disenrollment processes.
• Develops and implements care plans with members, caregivers, physicians and staff.
• Monitors care plan effectiveness, documents interventions and adjusts as needed.
• Promotes integration of services across LTSS and behavioral health to support member care.
• Assesses medical necessity and authorizes waiver services; collaborates with care teams.
• Engages with ICT meetings for service approval/denial.
