Qualifications

• 2+ years in health care; 1+ year in care management or LTSS settings
• Licenses or credentials listed above must be active and unrestricted

Responsibilities

• Complete comprehensive member assessments within timelines and may conduct in-person home visits
• Coordinate waiver enrollment/disenrollment and develop care plans with members and care network
• Monitor care plans for effectiveness and adjust as needed
• Promote integration of behavioral health and LTSS services to ensure continuity of care
• Assess medical necessity and authorize waiver services; evaluate benefits and funding sources
• Facilitate ICT meetings for service approvals/denials and collaborate with the team
• Provide education and motivational support to members during contacts
• Identify barriers to care and assist with psycho/social, financial, and medical obstacles
• Travel up to 25-40% may be required per state/contractual requirements

Education

Licensed social work professional (LCSW/LMSW/APSW)

Licensed clinical counselor/therapist (LPCC/LMFT/LPC)

Healthcare related bachelor’s degree
