Job summary

Qualifications

• Bachelor’s degree in Social Work, Nursing, or related health care field including LPN.
• 2+ years of clinical case management experience.
• 1+ years of MS Office experience (Word, Excel, Outlook).
• Excellent organization and time management skills; strong communication.
• Reside in or near Washington County; valid US driver’s license with travel up to 75%.

Responsibilities

• Coordinate member care needs for medically complex members.
• Identify health risks, education, and prevention.
• Coordinate care across acute, home, chronic, and alternative settings.
• Collaborate with Case Management, Utilization Review, and Behavioral Health teams.
• Perform telephonic and face-to-face assessments per policy.
• Document in Health Plan software; maintain time-sensitive documentation.
• Advocate for members to resolve barriers to care.
• Provide education and referrals to community resources.
• Develop and maintain member-centered care plans.
• Monitor hospital encounters and educate to reduce frequency.
