Job summary

CVS Health is seeking a Clinical Case Manager in Behavioral Health for field-based work across Tarrant and Dallas counties, including Fort Worth and Arlington. The role requires 75% travel to meet members in person and deliver integrated behavioral health services.

The position emphasizes crisis intervention, care coordination, and collaboration with medical directors to optimize outcomes. A master’s in behavioral/mental health and an unencumbered license are required.

Qualifications

• Direct clinical practice experience post masters degree in hospital or ambulatory settings.
• Willingness and ability to travel up to 75% of the time across Tarrant/Dallas counties.
• Unencumbered Behavioral Health clinical license in the state (LCSW/LPC/LMSW/LMFT).
• Case management and discharge planning experience preferred.

Responsibilities

• Assess referred members' needs/eligibility and determine care resolution by evaluating benefit plans and available programs.
• Apply clinical judgment to reduce risk factors and address complex indicators in care planning.
• Perform crisis intervention with members in behavioral health or medical crisis and refer for thorough assessment.
• Provide crisis follow-up to ensure appropriate treatment and services are received.
• Use standardized guidelines and case management plans to ensure proper administration of benefits.
• Collaborate with supervisors and medical directors to overcome barriers and present cases for multidisciplinary review.
• Identify and escalate quality of care issues through established channels.
• Engage with medical/behavioral health professionals to influence member care.
