Job summary

The Family Resource Network is seeking a Support Coordinator to help individuals with disabilities access needed services across New Jersey. The role requires travel and a flexible schedule, including evenings and weekends, and reports to the Support Coordinator Supervisor at the corporate office.

You will develop plans of care, coordinate with families, and connect clients with community resources while maintaining ongoing contact as plans are put into place.

Qualifications

• Bachelor’s degree in a relevant discipline is required.
• Proficient with MS Office applications (Outlook, Word, Excel, PowerPoint).
• Strong organization and communication skills with good time management.

Responsibilities

• Develops individual plans of care with input from the individual and family.
• Utilizes community resources and supports to help individuals and families.
• Maintains ongoing contact as plans are implemented and updated.
• Coordinates and oversees supports selected by individuals and families.
• Works as part of a regional support team and keeps stakeholders informed.
• Performs training and attends meetings as assigned.
