Job summary

Adaptive Home Health is seeking a Speech-Language Pathologist to provide in-home evaluations and therapy across a field-based Missouri City service area. You will own the SLP plan, deliver skilled visits, and work with a multidisciplinary team.

This role features a productivity-based pay model, schedule flexibility, and technology that simplifies charting and coordination, enabling you to focus on patient care and outcomes.

Qualifications

• Active Texas SLP license in good standing.
• Strong clinical reasoning and ability to operate independently in the field.
• Comfort identifying red flags and escalating changes to RN/MD.
• Strong caregiver education (teach-back, carryover planning).
• Strong communication and thorough documentation.
• Reliable follow-through on orders, frequencies and goals.
• Current driver's license and comfort with daily travel across territory.

Responsibilities

• Own outcomes, not just visits: manage the patient’s SLP plan and functional outcomes.
• Complete in-home SLP evaluations and deliver skilled visits.
• Assess and treat per orders and clinical indication.
• Build and execute a clear plan of care with measurable goals.
• Drive multidisciplinary coordination and align orders with care team.
• Coach patients and caregivers with practical strategies and routines.
• Document promptly and thoroughly.
