Job summary

Adaptive Home Health is seeking a Speech-Language Pathologist (SLP) to provide skilled in-home therapy across a field-based service area. You will assess, treat, and monitor communication and swallowing goals in patients' homes, coordinating with the care team to ensure continuity of care.

The role emphasizes measurable outcomes, caregiver education, and thorough documentation using Homecare Homebase (HCHB). A Texas SLP license and willingness to travel are required for this field-based position.

Qualifications

• Active SLP license in Texas or eligible for state licensure
• Strong clinical reasoning and independent field work ability
• Excellent caregiver education and teach-back skills
• Solid communication and thorough documentation practices
• Reliable follow-through on orders, frequencies, and patient goals
• Valid driver’s license with willingness to travel daily within territory

Responsibilities

• Own patient outcomes and the SLP plan, progression, and functional results.
• Perform comprehensive in-home SLP evaluations and provide skilled visits.
• Assess and treat aphasia, cognitive-communication issues, and dysphagia per orders.
• Develop and execute a measurable plan of care with functional goals.
• Coordinate with the care team and adapt orders as conditions change.
• Coach patients and caregivers with practical, actionable routines.
• Document promptly in Homecare Homebase (HCHB) with accuracy.
