Job summary

Easterseals Louisiana is seeking a Support Coordinator to oversee client services, coordinate resources, and ensure timely access to community supports. The role involves regular field visits to participant homes and collaboration with providers to plan and monitor services.

You will respond to changes in participant needs, complete required paperwork and annual assessments, and participate in weekly supervision and trainings.

Qualifications

• Requires CSWE-accredited social work degree with two years of post-degree coordination experience.
• Alternative RN license with two years of post-degree coordination experience.
• Human Service field degree with two years of post-degree coordination experience.
• Liberal Arts degree with 16 credit hours in psychology/education/etc and two years of coordination experience.
• Reliable transportation for home visits (about 50% field time).
• Easterseals Louisiana is a Drug-Free Workplace; background and driver's license checks required.

Responsibilities

• Complete quarterly visits and monthly contacts; generate required units and monitor participants.
• Conduct annual CPOCs according to funder timelines.
• Schedule quarterly meetings with participants and providers.
• Respond to participant changes and submit paperwork.
• Research and link services; communicate with providers.
• Attend weekly staff meetings and supervision.
• Support participants during emergencies including weather-related evacuations.
• Meet supervision and training requirements; mandated reporter per Louisiana Children’s Code Article 610.

Education

Bachelor’s or Master’s Degree in Social Work (CSWE-accredited)

Bachelor’s or Master’s Degree in Nursing (RN) licensed in Louisiana

Bachelor’s or Master’s Degree in a Human Service related field

Bachelor’s Degree in Liberal Arts with concentration in psychology/education/counseling-related fields
