Job summary

Adaptive Home Health is seeking a Speech-Language Pathologist (SLP) for a field-based, full-time role in Texas. You will deliver in-home evaluations and skilled visits, drive functional outcomes, and coordinate with the care team to support patient communication, cognition, and swallow safety.

You will be compensated on a productivity-based model—75 per point, with 28 points per week—alongside scheduling support and technology to streamline documentation and routing.

Qualifications

• Active Texas SLP license (in good standing).
• Strong clinical reasoning and ability to operate independently in the field.
• Ability to identify red flags and escalate changes in condition to RN/MD.
• Strong caregiver education and teach-back skills.
• Strong communication and documentation.
• Reliable follow-through on orders, frequencies, and patient goals.
• Valid driver's license and comfort with daily travel across assigned territory.

Responsibilities

• Own outcomes, not just visits: manage the SLP plan and functional outcomes.
• Complete in-home SLP evaluations and skilled visits.
• Assess and treat as ordered/indicated.
• Build and execute a plan of care with measurable goals.
• Drive multidisciplinary coordination with the care team.
• Coach patients and caregivers with practical strategies.
• Document promptly in Homecare Homebase (HCHB).
