Qualifications

• Master’s or Doctoral degree in Speech-Language Pathology.
• Current SLP license in the state of practice.
• CCC-SLP is preferred.
• Ability to work independently in home health settings.

Responsibilities

• Lead evaluation and treatment of patients with speech, language, cognition, and swallowing disorders.
• Develop and implement individualized plans of care to improve communication and safety.
• Provide evidence-based interventions using specialized tools.
• Assess home environments and recommend aids or equipment.
• Educate patients and caregivers on exercises and safety techniques.
• Monitor progress and adjust treatment plans as needed.
• Document care in the EMR with accuracy and timeliness.
• Collaborate with nurses, therapists, and care teams for coordinated care.
• Participate in case conferences and discharge planning.
