Job summary

TEEMA Group is seeking a compassionate Registered Nurse for a full-time home health and hospice role. You will lead an interdisciplinary team and provide high-quality, personalized care to patients and families in a field-based setting.

Responsibilities include comprehensive assessments, symptom management, caregiver education, and coordination with physicians. Must hold an active RN license and be able to travel within an established radius.

Qualifications

• Active, unencumbered RN license in the state of practice.
• Ability to pass mandatory background checks and health screenings.

Responsibilities

• Perform initial and ongoing home health and hospice assessments and develop individualized care plans.
• Deliver skilled nursing interventions and manage pain and symptoms.
• Lead and coordinate an interdisciplinary team to ensure holistic patient care.
• Educate caregivers on care plans, medication administration, and safety.
• Communicate with physicians, patients, and care team about changes in patient condition.
• Maintain accurate EMR documentation including visit notes and orders.
• Supervise care team members during visits and ensure standards of practice.
