Job summary

TEEMA Group is seeking a dedicated Registered Nurse to join our home health and hospice team in Williamston, NC. This full-time role involves leading an interdisciplinary team, conducting comprehensive clinical assessments, and managing patient care plans in the field.

You will educate caregivers, coordinate with physicians, and maintain complete EMR documentation while ensuring high-quality, compassionate care for patients and families in their homes.

Qualifications

• Active RN license in the state of practice.
• Willingness to travel within a 60-mile radius from the branch.
• Strong clinical assessment, patient education, infection control, and case management skills.
• Ability to pass background checks and annual compliance training requirements.

Responsibilities

• Perform initial and ongoing home health and hospice assessments, developing individualized care plans per physician orders.
• Deliver skilled nursing interventions and manage pain and symptoms in advanced illness or end-of-life care.
• Lead and coordinate care with LPN/LVNs, CNAs, and support staff for holistic patient and family care.
• Educate patients, family members, and caregivers on care plan execution, medications, wound care, oxygen safety, and disease management.
• Act as primary liaison between physicians, patients, and the care team; report changes to attending physicians as needed.
• Maintain accurate EMR documentation: visit notes, orders, and care plans.
• Conduct routine supervisory visits for care teams to ensure standards of practice and compliance.
• Participate in QAPI initiatives and uphold infection control and safety protocols.
• Assist with other duties as assigned.
