Job summary

CareSource seeks an Integrated Care Clinical Manager in Massachusetts to coordinate complex care for dually-eligible enrollees. You will perform assessments, lead the ICT, and collaborate with clinicians and community resources to improve outcomes.

The role requires MA RN licensure, nursing experience in care coordination, and ability to travel to member homes and centers. A strong focus on culturally sensitive, person-centered care is essential.

Qualifications

• Associates of Science degree in nursing from an accredited program.
• Registered Nurse with the ability to independently serve people with complex needs.
• Prior experience in care coordination, case management, or dual-eligible populations preferred.
• Medicaid and/or Medicare managed care experience preferred.
• Case Management Certification is highly preferred.
• Must have valid driver's license, vehicle and verifiable insurance; travel as required.

Responsibilities

• Provide in-home and community-based care management.
• Lead the interdisciplinary care team and develop holistic care plans.
• Coordinate with providers and community resources for seamless care transitions.
• Perform comprehensive assessments and update care plans as needed.
• Monitor enrollee utilization and prevent readmissions; ensure option to remain in preferred location.
