The Registered Nurse (RN) provides clinical oversight and coordination of care for clients receiving home and community-based services within their assigned area. The RN is responsible for completing initial assessments and reassessments, developing individualized care plans, overseeing clinical documentation and compliance, managing physician plans of treatment, and supporting the clinical needs of clients and caregivers.
Operating within the area-based pod model, the RN serves as a clinical resource to LPNs, PRN nurses, caregivers, and operational teams, providing guidance, collaboration, and escalation support as needed. While the role primarily focuses on clinical oversight, care coordination, and administrative responsibilities, the RN maintains flexibility to provide field-based services and coverage when operationally necessary to ensure continuity of care and compliance with regulatory requirements.
