• Reliable transportation, MI Driver's License & Auto Insurance.

• Conduct case opening assessments.
• Create and manage care plans.
• Perform medication set up and reconciliation.
• Conduct wound care visits.
• Ensure timely paperwork completion meeting compliance standards.
• Make supervisory visits with clients and field staff.
