Job summary

Care Finders Total Care, a Modivcare Personal Care Service, seeks a full-time RN Clinical Supervisor to support patients and home health caregivers across Union County. The role involves independent field work, nursing assessments, and clinical guidance to caregivers to ensure safe, high-quality home care.

Newly licensed RNs are welcome, and training and support are provided to help you succeed in a community-based setting.

Qualifications

• Active New Jersey RN license required.
• Current CPR certification required.
• Strong clinical assessment, communication, and patient education skills.
• Able to travel throughout Union County for patient visits.

Responsibilities

• Conduct nursing assessments and supervisory visits in patients' homes.
• Supervise, evaluate, and provide clinical guidance to CHHAs.
• Develop, review, and update individualized plans of care.
• Monitor patients' conditions and communicate changes to the care team.
• Provide clinical orientation, education, and training to caregivers.
• Collaborate with physicians and the care team to coordinate patient services.
• Maintain accurate clinical documentation in line with policies and regulations.
