Job summary

Acentra Health is seeking a Clinical Assessor, Mid to join our expanding team. The role involves determining Medicaid-funded PCS eligibility and assessing level of care to keep individuals in home and community settings.

The position requires a valid driver’s license and travel across locations, with travel expenses covered. We provide field-ready tech equipment for remote work and a driver-friendly schedule.

Qualifications

• Must be a licensed Registered Nurse in NC or compact.
• Proficient with Microsoft Office suite (Word, Excel, Outlook, Teams).
• Detail-oriented with strong organizational skills and time management.

Responsibilities

• Conduct PCS eligibility assessments using state-approved tools.
• Ensure services are provided on a needs-basis per unmet needs.
• Maintain privacy and dignity of individuals assessed.
• Meet established timeframes for initial, annual, and change-of-status assessments.
• Interview family members and informal caregivers during assessments.
• Assist beneficiaries in selecting PCS providers and participate in service plan reviews.
