Adaptive Home Health in Texas is seeking a Registered Nurse Case Manager (full-time) to deliver high-acuity home health care in the field. You will own patient assessments and documentation, emphasizing wound care and infusion therapy, while coordinating with physicians, families, and the care team.

You will advocate for patients, manage care plans, and drive timely follow-ups. A strong clinical judgment and ability to operate with autonomy are essential to succeed in this mission-driven role.

Qualifications

• Current Texas RN license
• 3+ years of clinical nursing experience
• Comfort with wound care and infusions
• CPR required (BLS)
• Current Texas driver's license and field-based work readiness
• Strong time management and organization in a high-autonomy environment
• Clear, professional communication with patients, families, and providers

Responsibilities

• Own patient assessments and skilled visit documentation including OASIS.
• Deliver high-acuity home health care with focus on wound care and infusion therapy.
• Build and maintain a clear plan of care through communication with physicians and care teams.
• Advocate for patients and ensure continuity of care across disciplines.
• Drive case management, follow-ups, and barrier resolution to progress toward goals.
• Coordinate services (PT/OT/ST/MSW/HHA) and update plans of care as needed.
• Administer medications and provide patient/caregiver teaching.
• Collaborate with clinical and operational teams to ensure safe, reliable care.
• Provide supervision to LVNs and HHAs where applicable.
• Participate in case conferences and in-services to align quality and outcomes.
