Job summary

Adaptive Home Health is seeking a Registered Nurse (RN) Case Manager to deliver skilled home health care across Texas. You will own patient assessments, manage high-acuity visits, and coordinate with physicians and families to drive optimal outcomes.

The role offers a field-based schedule with Monday–Friday hours, occasional weekends, and a productivity-based compensation model. Nurses with wound care and infusion experience are encouraged to apply.

Qualifications

• Texas RN license required.
• 3+ years of clinical nursing experience.
• Comfort with wound care and infusions.
• CPR (BLS) certification required.
• Current Texas driver’s license and field-based work.
• Strong time management and organization in autonomous environments.
• Clear, professional communication with patients and providers.

Responsibilities

• Own patient assessments and skilled visit documentation including OASIS.
• Deliver high-acuity home health care with focus on wound care and infusion therapy.
• Build and maintain a plan of care through communication with physicians, care team, patients, and families.
• Advocate for patients and ensure continuity of care across disciplines.
• Drive case management: care coordination, timely follow-ups, barriers resolution.
• Coordinate with internal teams to ensure plan of care is executed.
• Administer medications and treatments as ordered, and provide patient teaching.
• Supervise LVNs and HHAs when applicable.
• Participate in case conferences and training to align on quality and safety.
